Site name: Fill in with item name (max of 5 words, do not use ALL CAPS)
URL: Copy and Paste page URL of page on your site where item should be found.
Descritption of site: Fill in with brief item descritption.
Link to your banner: URL/Link to your product image (image should be no bigger then 300x300 pixels, preferably a photobucket image)
Rest of the form: Fill in as required (like any other top site)
